Please ensure you give your child's DOB, grade and relevant info in the email requesting the form.

Once you receive the form, kindly fill in the form and email it to: parksidekidsacademy@gmail.com or alternative hand it in at the preschool.

Ensure you have sent through the following:

• Proof of admin registration fee paid with your child's name and grade as a reference

• Copy of birth certificate

• Copy of immunisation records
